Certificate of Insurance and Contractor Form
 

All exhibiting companies must submit a Certificate of Insurance (COI).  See requirements below.  Return COI to erika@theaestheticsociety.org.

Certificate of Insurance (COI) 
The Certificate of Insurance MUST be for $2,000,000 and list BOTH The Aesthetic Society and The Aesthetic Foundation under Certificate Holder.  This must be emailed to Erika Ortiz-Ramos no later than March 1, 2024.

PLEASE NOTE:  The description box MUST contain the name of the insured company as well as the insurer and the exhibiting company's name and/or booth number. If submitting a COI for multiple clients, please list all of the clients in the descriptions area.

Additional insured must be: Freeman and the Vancouver Convention Centre.

Exhibitor Appointed Contractor (EAC)
If an exhibitor wishes to use a service firm other than Freeman to install or dismantle their booth, an Exhibitor Appointed Contract (EAC) form, along with a certificate of insurance must be submitted no later than March 1, 2024 for EACH contractor you plan to use.  So if you have one for your install/dismantle and one for your AV, you must complete two forms and each of them must submit their Certificate of Insurance.
