Certificate of Insurance and Contractor Form
All exhibiting companies must submit a Certificate of Insurance (COI). See requirements below. Return COI to erika@theaestheticsociety.org.
Certificate of Insurance (COI)
The Certificate of Insurance MUST be for $2,000,000 and list BOTH The Aesthetic Society and The Aesthetic Foundation under Certificate Holder. This must be emailed to Erika Ortiz-Ramos no later than December 31, 2024.
The address must be: 11262 Monarch Street, Garden Grove, CA 92841
Additional insured must be: Freeman and the Austin Convention Center.
PLEASE NOTE: The description box MUST contain the name of the insured company as well as the insurer and the exhibiting company's name and/or booth number. If submitting a COI for multiple clients, please list all of the clients in the descriptions area.
COI's without this information will not be accepted.

Exhibitor Appointed Contractor (EAC)
If an exhibitor wishes to use a service firm other than Freeman to install or dismantle their booth, an Exhibitor Appointed Contract (EAC) form, along with a certificate of insurance must be submitted no later than December 31, 2024 for EACH contractor you plan to use. So if you have one for your install/dismantle and one for your AV, you must complete two forms and each of them must submit their Certificate of Insurance.
