Instructions
1. Before verifying using proof of vaccination, you must:
Be fully vaccinated with the following FDA authorized vaccines:
- Pfizer (fourteen days after the second dose),
- Moderna (fourteen days after the second dose), OR
- Johnson & Johnson (fourteen days after the first and only dose),
AND
Have access to an acceptable, legible proof of vaccination.
2. Sign in to your ID.me account.
If you do not have an ID.me account, you can create a new one and verify your identity using one of the options below:
- Phone number
- Driver's license, state ID, passport, or passport card
3. Select Upload proof of vaccination to begin verifying your COVID-19 status.
4. Enter the following information and select Continue.
- Type of FDA authorized COVID-19 vaccination you received
- Date of your first dose
- Date of your second dose
5. Select one of the following acceptable proofs of vaccination.
- CDC COVID-19 vaccination record card,
- State immunization registry record, OR
- Health provider vaccination record (e.g., MyChart).
6. Upload proof of vaccination as a photo or PDF file.
If you are verifying with your CDC COVID-19 vaccination record card, make sure to upload photos of both the front and back.
Select Retry if you see a "We couldn't verify your information" message on your screen. This will take you back to Step Three. Make sure your photo is clear, well lit, all the information is visible, all four corners are visible, and the photo is not blurry.
7. Review and confirm your information, then select Continue.
8. Complete your COVID-19 Status Verification and select Allow and continue.
This will allow ID.me to share your identity and COVID-19 Status Verification with SEMPA.
9. When checking in at SEMPA 360 you will be asked to show your verification email from ID.me with the event name or the verified COVID-19 Vaccine ID found in your ID.me account under My IDs.
