Achievable Health (formerly Achievable Foundation) is a nonprofit community-based organization whose mission is to provide high quality, integrated health care to individuals with intellectual and developmental disabilities, their families, and other vulnerable populations. Through our health center we aim to provide high quality, integrated, equitable, whole-person care to over 2,400 community members annually. Our vision is to change the face of healthcare for our vulnerable communities.
