Insurance and Contractor Form
 

Exhibitor Appointed Contractor
If an exhibitor wishes to use a service firm other than Freeman to install or dismantle their booth, an Exhibitor Appointed Contract (EAC) form, along with a certificate of insurance must be submitted online no later than March 1, 2023.  Please email erika@theaestheticsociety.org for a copy of the EAC form.

 
Certificate of Insurance
The Certificate of Insurance MUST be for $2,000,000 and list BOTH The Aesthetic Society and ASERF under Certificate Holder.  This must be uploaded along with the Exhibitor Contractor form.  Please log in here using the password provided on your Booth Confirmation email to complete the form. If you cannot locate the password, please email erika@theaestheticsociety.org.

PLEASE NOTE:  The description box MUST contain the name of the insured company as well as the insurer and the exhibiting company's name and/or booth number. If submitting a COI for multiple clients, please list all of the clients in the descriptions area.
